Position Summary
The Resident Care Coordinator delivers quality assisted living services focused on maximizing resident’s individual capabilities in accordance with all laws, regulations and Century Park standards. Reports to Resident Care Director (or Assistant Resident Care Director).
Qualifications
- Must possess a high school diploma or equivalent
- Certified Nurses Aide (CNA) certification, or Med Tech or QMAP certification preferred
- Must have minimum of one (1) year experience in senior services
- Must have CPR certification upon hire or obtain during Orientation. Certification must remain current during employment.
Primary Job Responsibilities
- Provides assistance with Resident Care staff scheduling, including coverage for planned and unplanned absences
- Provides assistance with scheduling meetings with families, healthcare providers, home health, etc.
- Assists new hires with on the job training and onboarding in the Resident Care department
- Performs duties as care aide, QMAP, Med Tech or CNA as needed and as licensure allows
- Assists with receiving physician orders, transcribing to medication sheets and charts appropriately
- Provides assistance with communication with residents, families, physicians, hospice, and vendors
- Provides assistance with entering resident information in the Electronic Health Record (HER) system
- Assists with coordinating wellness clinics, flu clinics, ancillary appointments (i.e. podiatry), and transportation needs
- Provides assistance with lab orders, preparing new resident charts, and proper disposition of residents charts who have moved away or expired
- May assist with medication management refills, delivery to med carts, expired/discontinued meds (as State licensure allows)
- Assists resident with care needs when necessary (as State licensure allows)

- Must have patience, tact, cheerful disposition, and enthusiasm as well as a willingness to handle difficult residents
- Must possess the ability to make independent decisions when circumstances warrant such action
- Must perform efficiently and effectively with little to no supervision
- Maintains professional working relationships with all associates, vendors, etc.
- Maintains confidentiality of all proprietary and/or confidential information
- Displays integrity and professionalism by adhering to Century Park’s Code of Ethics and completes all required compliance training
