Harrisburg Family Health Care is seeking a Family Nurse Practitioner to join our growing team in Augusta, Georgia. This is an exciting opportunity to join a primary care practice known for providing compassionate, holistic, patient-centered primary care to individuals regardless of their ability to pay. The practice environment is team-based with a focus on social determinants of health and integrative services such a behavioral health. The ideal candidate will have experience working with diverse patient populations in an interdisciplinary care setting.
Looking for candidate for part-time position with interest in transitioning to full-time.
Duties
Act within professional boundaries to provide compassionate and comprehensive care for patients from initial history taking, clinical assessment, diagnosis, treatment, and care evaluation.
Demonstrate critical thinking in the clinical decision-making process and provide expert care for patients through the formulation of appropriate and personalized care plans for patients in consultation with the interdisciplinary team, specialty physicians, community agencies, and other volunteer professionals.
Participate in educational opportunities with students from multiple disciplines, quality improvement and research projects, and collaborate on innovative patient care programs.
Work collaboratively with the organizational leadership and the interdisciplinary team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required.
