We are seeking a Registered Nurse Case Manager to join our team. The successful candidate will be responsible for providing patient-centered care and managing the overall health of their assigned patients. This includes assessing the patient's medical condition, developing a plan of care, monitoring progress, coordinating with other healthcare providers, and providing patient education. The ideal candidate will have excellent communication and organizational skills and a commitment to providing quality care.
Responsibilities:
- Develop and implement patient care plans.
- Monitor patient progress and evaluate outcomes.
- Provide patient education and counseling.
- Coordinate with other healthcare professionals to ensure quality patient care.
- Document patient care services in accordance with regulatory standards.
- Manage a caseload of patients and provide appropriate follow-up care.
- Assess the need for additional resources or services for patients and families.
- Maintain accurate records of patient care services.
Job Type: Full-time
Pay: $34.00 - $37.50 per hour
