Haven is a thriving home health agency seeking nurses that want to make a difference and be part of a team of health care professionals that are committed to quality nursing care. We provide all levels of care to patients in our community, who need our help to recover from a myriad of ailments. We specialize in complex and high-tech nursing for wound care, intravenous therapies and rehabilitation. We are located in Long Beach (Signal Hill) and service both LA and Orange Counties.
All new clinical employees go through an extensive orientation process, that is based on the principals of observing, being observed and independent performance. All new employees are given the option of determining when they are ready for independent performance. We will provide all training needed and facilitate proficiency in tasks for clinical staff.

RESPONSIBILITIES:
- Admits patients to agency
- Provides follow up visits for patients when admissions are not available
- Coordinates admission with case manager and intake staff
- Notifies the doctor of any changes in condition and requests new orders as needed
- Advises the Director of Nursing on current events, problems, and Agency's operations on a daily basis, or as needed.
- Keeps informed of new developments as related to the Agency in regard to State and Federal regulatory agencies.
- Assures quality of patient care by working closely with the Utilization Review/Quality Assurance Coordinator and follows through on corrective action when appropriate.
- Acts as a liaison among Agency personnel, client/client’s family members and physicians when necessary
- Attends meetings and trainings as requested or required
- Participate in on-call rotation
- Adheres to Agency's policies and procedures
- Performs other duties as assigned
QUALIFICATIONS:
- Active license the State Board of Nurses as a registered nurse.
- Ability to work independently with minimal supervision.
- TB Test and Physical exam six months prior to employment or within 14 days of the date of employment and a TB thereafter annually. A chest x-ray, because of a positive TB PPD test and physicians recommendation for x-ray.
- Valid Driver’s license and auto insurance
- Satisfactory references.
