Job Summary and Qualifications
The Medical Social Worker for our department will provide clinical expertise to ensure all patients receive high-quality, efficient care. Our dynamic team works hard to support each other and to ensure the delivery of excellent patient care. In this role you will have the ability to positively impact and improve lives.
What you will do in this role:
- You will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management.
- You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
- You will identify potential barriers to patient throughput and quality outcomes
- You will evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.
- You will act as a liaison between the facility and resources external to the organization.
- You will help with the facilitation and implementation of timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care
- You will provide crisis intervention and support to patient/family support systems.
What Qualifications you will need:
- Licensure as a LMSW is required, LCSW is highly preferred
- Master’s degree in Social Work Required
- 3-5 years of experience in hospital patient care preferred
- This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
