Admission Coordinator

Healing Care Hospice is seeking an organized, dedicated, and responsible multi-tasker to serve as our Admission Coodinator for our Branch Office in Downey. The position is Alternating Sat & Sun from 8am - 5pm.

Candidate must be very detail-oriented and be willing to juggle multiple demanding tasks simultaneously.

Responsibilities include but are not limited to:

• Receive referrals from physicians, hospitals, facilities and/or families and initiate the intake process.
• Enter new patient records in system ensuring accuracy of all patient's demographics information.
• Verifying insurance and benefit periods (processing thru Medcare/Medi-Cal system and also calling insurance companies to verify eligibility)
• Obtaining authorization (Medical groups, IPA and/or insurance)
• Responsible for obtaining admissions documents (Consents, POLST, Admission forms etc.)
• Scheduling Clinical Liaisons to meet with families for the initial process of hospice.
• Scheduling admission nurse to admit new patient's
• Assigning teams (nurse, MSW, SC and HHA) when patient's are admitted
• Maintains weekly referral/admissions reports

Excellent customer service skills are a MUST

Strong computer and phone skills are required (Word, Excel and Outlook)

Spanish-speaking is a MUST; Bilingual.

• Downey, CA 90241: Reliably commute or planning to relocate before starting work (Required)
