General Description
Provides support to the Population Health Department by answering and triaging member and provider calls coming in through HPSM’s health education phone line.
Duties & Responsibilities
Essential Functions:
- Communicate directly with members and providers that call HPSM’s health education phone line.
- Complete intakes to assist in connecting members and providers to appropriate team members for accessing Health Promotion programs and/or Language Assistance Services.
- Route all intakes to appropriate team members to close the loop on Health Promotion program and/or Language Assistance Services requests.
- Triage calls to other member-facing teams when appropriate.
- Document all calls with members and providers in appropriate systems.
- Support in reviewing and routing health education phone line voicemails.
- Educate HPSM staff as needed regarding Population Health and Health Promotion programs available to members.
Secondary Functions:
- Assist with special projects as needed.
- Cross train, as requested, to provide back-up to other departmental functions.
- Perform other duties as assigned.
Requirements

Education and experience
- A High School Diploma; Associates Degree in a health related field is desired.
- One (1) year of experience in a health care delivery system working directly with patients, or in a managed care plan working directly with members is preferred.
- Working in a heavy customer service-based work environment dealing with a multicultural population.
Knowledge of:
- Personal computers and proficiency in Microsoft Office Suite applications, including Outlook, Word, Excel, Access, and PowerPoint.
- General office practices and procedures.
- General office equipment including a personal computer, fax, phone, copier, etc.
Ability to:
- Work cooperatively with others.
- Work as part of a team and support team decisions.
- Communicate effectively, both verbally and in writing.
- Adapt to changes in requirements/priorities for daily and specialized tasks.
- Utilize a personal computer and the range of Microsoft Office products including Word, Access and Excel.
- Work collaboratively within a multidisciplinary team.
- Communicate effectively, verbally and in writing.
- Provide excellent customer service.
- Problem-solve to resolve day-to-day issues.
- Organize and maintain accurate records and files.
- Organize work, set priorities, meet critical deadlines, and follow up on assignments.
Skills:
- Bilingual proficiency highly preferred
Salary
The starting salary range is $22.09 - $28.72, depending on the candidate’s work experience.
