Job Title: MDS Coordinator
Job Summary:
HealthBridge is seeking a highly organized and detail oriented MDS Coordinator to join our team of healthcare professionals. As an MDS Coordinator, you will play a vital role in ensuring the delivery of high-quality care to our residents.
Responsibilities:
- Conduct thorough and accurate assessments of residents' health status, functional abilities, and psychosocial needs using the Minimum Data Set (MDS) tool.
- Update and maintain resident data in accordance with state and federal regulations and company policies.
- Collaborate with interdisciplinary teams to develop comprehensive care plans based on assessed needs and goals.
- Regularly review and update care plans to ensure they reflect changes in the resident's condition or preferences.
- Monitor and track resident outcomes and make necessary adjustments to the care plans as needed.
- Ensure compliance with state and federal regulations regarding data collection, reporting, and assessment timelines.
- Provide education and training to staff members on proper assessments techniques, documentation requirements, and regulatory compliance.
- Participate in quality improvement initiatives and contribute to identifying areas for improvement in resident care.
- Sever as a resource for staff members, families, and external stakeholders regarding MEDS assessments and related topics.
- Stay up-to-date with industry changes and best practices related to MDS assessments and care planning.
Qualifications:
- Valid nursing license (RN/LPN) in Michigan.
- Minimum 2 years of experience in long term care or rehabilitation settings.
- Proficient in conducting MDS assessments and familiar with MDS coding guidelines.
- Strong knowledge of state and federal regulations related to long-term care and MDS assessments.
- Excellent communication and interpersonal skills to effectively collaborate with interdisciplinary teams.
- Attention to detail and accuracy in documentation and data entry.
- Ability to prioritize and manage multiple tasks within established deadlines.
- Proficient in using electronic health record (EHR), (PCC), and other related systems.
- Strong problem solving skills and ability to make critical assessment based on available data.
