Position Summary: HealthFlex is seeking a compassionate and experienced Registered Nurse (RN) - Home Health, Case Manager. By joining our 5 star rated clinical team; you will be providing skilled services to help patients regain their independence in the comfort of their own home.
Schedule: Full Time, Part Time or Per Diem
Locations: We have needs across all of our territories in the following counties, Alameda, Contra Costa, Santa Clara, San Mateo, San Francisco, Marin, Sonoma, Napa and Solano.
Responsibilities:
- Perform a full assessment of patient & family upon initial visit
- Use assessment data to develop an advanced plan of care
- Work with patients and family one on one to achieve improvement and/or stability.
- Case manage patients care episode
- Prepare clinical notes, orders and charting in a timely manner
- Collaborate with physicians and agency staff to deliver the best care possible

Requirements
- Current and unrestricted CA RN License
- 1 year RN experience, Home Health or Hospice experience preferred
- Current AHA CPR certification
- Valid Driver's License
