Position Summary:
The Case Manager works under direct supervision of the Site Manager. The Case Manager in this position provides direct disease and intervention awareness to the public, as well as education, counseling and referral services to individuals that are at risk of STD/HIV infections.
Essential Duties and Responsibilities:
Requires a professional and interpersonal skill to interact with clients, and healthcare professional
Requires knowledge of varied social, economic, cultural, and lifestyle characteristics and the ability to communicate with clients as well as target groups of diverse individuals.
Performs Case Management activities as directed to help identify individuals that are at risk STD/HIV infection.
Completing the HMD Case Management Form, 1628, and Hepatitis C Risk Assessment Form.
Conduct client registration in the EMR and complete Quest Lab requisitions.
Perform phlebotomy, urine collection, finger sticks, rapid testing, and pregnancy tests for proper lab processing.
Travel to different site location to conduct STD/HIV Screenings to help identify individuals at risk. These events can occur before, during or after normal working hours including weekends or holidays.
Handles sensitive information with high level of confidentiality
Performing patient intake, requesting labs, scheduling appointments and follow ups and preparing client files.
Assist in insurance pre-authorization to obtain approval for medications.
Conduct assessment of risk factors to contributes to HIV/HEP C and STD's.
Practice safety, environmental and infection control methods.
Assist with direct patient care procedures including LMP's, medical histories, patient charts, vital signs and medical examinations.
Work and complete tasks in EMR on a timely manner.
Perform standard operations procedures as outlines by IHMD
Requirements:
1 year of Disease Intervention or related field and/or equivalent professional experience.
1 years in Case Management experience
1 year phlebotomy experience
1 years of extensive working knowledge of STD's/HIV
Solid knowledge of EMR/EHR systems and telehealth.
Perform Specimen collection/ Blood and Urine for STD/HIV Testing
Excellent communication and interpersonal skills
Strong analytical and problem-solving skills
Preferred Requirements:
500/501 HIV Training
Proof of continued education such as certifications
Hepatitis Testing Training
Strong knowledge of PrEP (Pre-exposure prophylaxis)
Highly organized and a proven strategic thinker with ability to engage and motivate teams
