Patient Care Manager is a compassionate, flexible, and organized RN with leadership abilities to be a patient care manager. This is an office position.
As a member of the Patient Care Manager Team, you will serve as a critical liaison between the benefit of hospice services and potential patients, their loved ones and caregivers, and our healthcare partners. Contributing to patient access to care at Heart of Hospice will not only bring you purpose in and satisfaction with your work, but will also welcome you into our dedicated, skilled, and mission-driven team.

The Position:
Our Patient Care Managers lead a multi-disciplined team and care for 50-60 patients. The PCM is responsible to plan, organize, and direct all patient care services, High level of visibility in the community and facilities his/her team services
The Requirements:
- Current CPR certification
- Registered nurse with current licensure with no restrictions to practice professional nursing in the state.
- Graduate of an accredited school of nursing with a BSN degree is preferred
