Responsible for assessing clients, documenting observation and assessment data, evaluating outcomes, and implementing the nursing plan of care. Implements the medical plan of treatment and follows physicians' orders.

Physical Demands
Individuals must have the ability to stand and work or have mobility through mechanical assistance. Must be able to lift and carry items weighing up to 20 pounds. This position requires the stamina and physical ability to provide personal care for non-ambulatory clients, use a Hoyer lift, maneuver wheelchairs and transport clients to community events and social activities.
Work Location
Home-like and office settings with varying degrees of background noise. Light and ventilation as found in a typical home or office setting. Hazards may exist if the employee fails to use proper body mechanics. Additionally, this position is required to work in client homes with a variety of climate conditions.

- Current GA RN licenses;
- Current CPR/First Aide certification;
- Current TB screening
- May be required to have current certification in Advanced Cardiac Life Support (ACLS) and/or Pediatric Life Support (PALS).
- 3 years experience providing skilled care to the medically fragile

Key Responsibilities
- Monitors individual at all times, following all CHC and agency safety protocols
- Transports clients to activities, services, appointments and general community per ISP, CHC and agency
- Provides skilled care (G
- Tube, meds, etc.) to individuals based of the individual's care plan
- Documents activities and services accurately and timely according to CHC and agency regulations.
- Monitors general health conditions of the individual and notifies supervisor of any medical issues or if treatment is needed
