Nurse Case Manager needed for our Companion agency. You must have experience in both homecare and geriatrics. Knowledge of Clear Care and computer savvy a must. Job entails intake of new clients, visiting clients, creating and monitoring service plans, scheduling companions, meet and greets for new Companions, continuous training of employees to ensure competence of field staff, working with LTC insurance companies on behalf of clients. Anticipates, identifies and reports all potential risks in a timely manner. Managing the onboarding process and orientation of new field staff. Reports to CEO on program status.

Pay: $30.00 - $40.00 per hour
