Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary
The Center of Hope/Prescription for Hope Social Worker provides acute crisis intervention services, information and referral services, and community outreach and education to primary and secondary victims of violence, abuse, and neglect at Eskenazi Health. This position requires prompt assessment of the patient's bio-psycho-social needs, in a fast-paced Level I trauma center as a member of a multidisciplinary team. Position will also be responsible for providing information and training to community partners and to general public through community outreach. Bilingual is a plus!
Essential Functions and Responsibilities
- Identify and coordinate services throughout Eskenazi Health to meet the acute needs of primary and secondary forensic patients (Center of Hope) or other trauma patients (Prescription for Hope)
- Problem solving techniques, critical thinking skills, decision making
- Provide care appropriate to meet the age & cultural needs of the patients served
- Demonstrate knowledge of principles of growth & development of the life span
- Serve as a resource for all health care providers in the coordination & management for the care of primary & secondary forensic patients.
- Provide crisis intervention to primary & secondary forensic or other trauma patients.
- Facilitate the patient’s choice to prosecute by providing support & referrals to the criminal justice system.
- Identify resources & make referrals for health care & long-term therapy needs for primary & secondary forensic patients.
- Facilitates a variety of individual, family & group advocacy & therapeutic interventions.
- Actively serve on community collaborative groups/panels (such as the Marion County Sexual Assault Response Team)
- Collaborate with clients (and supervisor/team) to prepare individual plans of care to address assessed needs.
- Assist in completing & filing applications for orders of protection.
- Regularly evaluates & updates the progress & revise care plan at intervals determined by guidelines; consulting with client, family & team in this process.
- Continuously evaluates clients’ mental status & trauma-recovery needs, reporting progress to the team
- Identifies community resources & assists clients in their access to these resources
- Provides indirect service to external agencies & community through consultation/education
- Attend court or other legal meetings & provide support for clients who testify or attend court hearings.
- Strive for improved patient care with a focus on positive outcomes
- Participate in root cause analysis of forensic issues at Eskenazi Health as needed
- Seek grant funding and implementation of grant projects to sustain & further develop the Center of Hope program
- Complete applications for victims assistance funds for primary & secondary funds as applicable
- Collaborate with Prescription for Hope Violence Intervention Specialist, the team &/or Prescription for Hope Program manager to assist in managing barrier buster funds requests
- Participate in critical incident stress debriefing activities as needed.
- Coordinate program-related educational offerings in both the professional & community settings, including networking to create opportunities for educational programs
Job Requirements
- Master's degree social work, LSW required with LCSW required within 2 years of hire.
- Ability to fulfill duties noted in area specific competencies (Adult, Acute, Youth, Family Systems, Domestic Violence, Sexual Assault, Other trauma-related competencies)
- Upon approval if candidate does not have licensure, a plan to obtain licensure within three (3) years may be accepted in lieu of current licensure
- Experience in a healthcare or community setting
- Demonstrated knowledge of community resources
- Valid Indiana driver's license that meets Eskenazi Health's driving policy requirements
