JOB SUMMARY: Responsible for maintaining and expanding Information and Referral program to increase living options for elders by assessing their needs and providing resources to maintain the living arrangement of his/her choice. Develop and promote Caregiving Initiatives.
AUTHORITY: This position reports to and is supervised by the I&R Supervisor
EDUCATION
REQUIREMENTS: Bachelor’s Degree preferred
EXPERIENCE: Experience in human service delivery, preferably with elder programs. Strong written communication, public speaking and computer skills required. CIRS eligible.
PRIMARY FUNCTIONS AND RESPONSIBILITIES:
I. Maintain and expand Information and Referral localization by:
1. Providing Information and Referral services, using the database and other resources, to callers of Highland Valley Elder Services (HVES).
2. Completing intakes, assisting Washington House callers and residents seeking Home Care, Home Delivered Meals, Nursing Home Screenings, Adult Day Health Screenings, Money Management and Protective services.
3. Training, assisting and updating HVES staff to understand and access pertinent resources to enhance utilization of community services.
4. Evaluating the effectiveness of the referral system consistent with quality standards to include consumer feedback and satisfaction regarding both the resources offered and the department.
5. Conducting outreach and marketing of Information and Referral Program both internally and externally.
6. Conducting Options Counseling visits to provide education around available resources to enable consumers to reach identified goals.
7. Providing support to SHINE and BCAA program staff by assisting with coordination of appointments and providing resource information to callers around these programs.
8. Providing recreational and community activities as required.
II. Promote Caregiving Initiatives by:
1. Working to mobilize the agency’s programs under the Caregiving Initiative by identifying caregiving issues and developing potential resources for caregivers.
2. Provide interdisciplinary care management to link and coordinate resources with elders.
3. Participate in Highland Valley’s development of comprehensive community programming efforts.
4. Participate in training and development activities to keep up with knowledge/skills leading to strong performance in this position.
5. Participate in required quality performance initiatives within program area.
6. Perform all other tasks as assigned by supervisor to meet agency needs.

a. Must be able to read and write English proficiently
b. Ability to communicate effectively both verbally and in writing
c. Basic typing skills (50 wpm) and strong data entry skills
d. Strong computer skills including thorough working knowledge of Windows 7
a. and MS Office
e. Excellent problem solving and analytical skills
f. Highly organized, detail-driven, self-motivated, with ability to work
g. independently as well as part of a team.
h. Ability to work under pressure to meet deadlines
2. Environmental Conditions
a. Work is 90% indoor office work; 10% outdoor (i.e. travel to attend required meetings, presentations and trainings)
3. Physical Requirements
a. While performing duties of position, employee will be required to sit/stand for
extended periods.
b. Working knowledge of computers applicable software and databases and
capacity to see computer screen and read written material.
c. Capacity to communicate verbally with staff/supervisors, consumers and
community.
d. Capacity for the fine manipulation with use of office equipment.
e. Capacity for occasional reaching, lifting or bending.
f. Capacity to lift up to 25 lbs.
4. Work Environment Expectations
a. Available to work an hourly position with primary hours daily M-F, 9AM-5PM.
