This team member provides direct nursing services in the client's home, directing care with a multi-disciplinary care team. The ideal candidate has the ability to work in an independent setting, educating clients and their families to promote independence in the home environment. Experience with catheter management, wound care, IV therapy and disease process education is a plus.
