JOB SUMMARY:
This position is responsible for providing personal care and delegated nursing services designed to maintain the residents’ physical and emotional wellbeing.
EMPLOYMENT RELATIONSHIP:
This position is an “at will” position and the employee and employer are free to sever the relationship at any time with or without cause. A minimum of 2 week notice of the intent to resign is requested for this position.
Experience/Educational Requirements:
- High school diploma or equivalent preferred.
- Must receive a “not disqualified” criminal background result from Department of Human Services and may not be on the OIG exclusion list.
- Must be screened for TB.
- Current Driver's License preferred.
Special Skills/Training:
- Ability to follow written and oral instructions.
- Receive a minimum of 8 hours training in direct care skills per year through Hillcrest or outside resources approved by registered nurse or facility manager.
- Ability to work under intermittent supervision.
- Pass practical, written, or oral exams on direct cares given by registered nurse.
Primary Responsibilities:
- Graduate of a recognized nursing assistant program and in good standing with the Minnesota NA/registry; OR successful completion of our agency’s orientation and training program for unlicensed personnel and has been determined competent to provide services and follow our agency’s procedures by our RN.
- Must be 18 years of age or older.
- Adaptability and flexibility in dealing with changing situations and in ability to remain calm when dealing with residents and their families, with a positive and pleasant and kind tone of voice.
- Ability to demonstrate good judgment and observation skills.
- Ability to effectively communicate with peers and supervisors and residents.
- Ability to deal tactfully and compassionately with residents, their family members, and other staff and to exhibit good customer service skills at all times.
- Ability to read, write, comprehend and follow verbal and written instructions, including medical terminology.
- Ability to work independently and to use good problem solving skills.
- Ability to meet the physical demands of the position, with or without accommodation.
- Ability to work flexible times, including weekends, evenings, nights and holidays.

- Must work as a team member with other personnel, supervisors, LPN, and RN.
Examples of Duties:
- Assist residents with personal cares as identified in the assignment sheet/client care plan. These cares may include all or some of the following: assisting with dressing, grooming and oral cares, shampooing of hair, assisting with showering or bathing, rubbing with lotions, fingernail cares and foot cares if instructed to do so by an RN, redirection of a confused resident, and other cares as assigned.
- Assisting residents in the use of the commode, urinal, toilet and with adult incontinence products as assigned.
- Assisting with ambulation and transfers, including proper use of transfer belt and assistive devices and proper body mechanics to avoid injury.
- Assist in the use of hearing aids and glasses for residents.
- Medication reminders, assistance or administration as delegated by RN.
- Prepare meals as directed, following special meal plans if instructed.
- Take and record temperature, blood pressure, pulse and respiration as instructed. Report changes or concerns to RN.
- Notify RN of any complaints or concerns raised by the resident or the resident’s family or representative.
- Help each resident to reach and maintain his/her highest level of performance and independence. Must promote resident’s independence by offering choices and fostering self-help skills.
- Establish rapport with clients, lead client in activities as assigned and provide socialization and companionship to residents while performing tasks.
- Assist with treatments as delegated by RN according to the client’s individualized care plan.
- Observe client and report to RN changes in resident’s physical and emotional condition.
- Assist co-workers with daily responsibilities when needed.
- Organize and prioritize assigned work during scheduled shift to complete assigned tasks.
- Document services provided accurately and consistently with company policies and complete all required paperwork in a timely and legible manner.
- Attend all required in-service training, as well as staff meetings and other company events.
- Follow company procedures for universal precautions when performing assigned tasks.
- Maintain a clean environment for residents by performing house cleaning duties as assigned.
- Assist with transferring of residents, monitoring of blood glucose and other services as directed by the RN.
- Follow company dress code and wear name badge while at work and maintain good personal hygiene and grooming.
- Follow company policies regarding safety rules and requirements. Report any issues or concerns about safety or any injuries immediately to supervisor.
- Responds to emergency situations according to defined policies and procedures.
- Maintains CONFIDENTIALITY regarding all client/work-related matters.
WORKING CONDITIONS
- Primary responsibilities will be conducted in facility with clean, well-lit, climate-controlled areas such as resident rooms, hallways, offices, dining areas and common areas.
- Exposure to body fluids, infection, odors and behavior of clients that may be aggressive, combative or demanding.
- The noise level is usually moderate.
PHYSICAL DEMANDS
- The hours as assigned, scheduled, or mutually agreed upon. Occasionally called upon to work on short notice.
- While performing duties of this job, the employee is regularly required to use hands and fingers to examine clients, handle or feel objects, tools, controls. In addition, the job requires walking, bending, stooping, pushing, pulling, kneeling, reaching over head and lifting up to 50lbs.
- Must have adequate vision and hearing necessary to perform job functions.
