POSITION SUMMARY:Reporting to the Director of Clinical Services, the Physical Therapist provides skilled physical therapy services to patients in their home according to a written physician’s plan of care.EDUCATION AND QUALIFICATIONS: Doctorate degree from an accredited Physical Therapy program, or Bachelor/Master’s degree, with equivalent experience Current Florida Physical Therapy license Minimum of two years of experience providing physical therapy services to in-home patients Current CPR certification Valid driver's license and automobile insurance in the State of Florida; must have reliable transportation to be able to meet visit schedule
ESSENTIAL JOB RESPONSIBILITIES: Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary. Complete assessments, testing, evaluating and re-evaluating for muscle/nerve joint function in a timely manner.
Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests. Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity.
Consults and familiarizes staff regarding the established plan.
SKILLS AND COMPETENCIES: Basic computer skills including use of email and word processing (Microsoft Outlook and Word or equivalent programs) Strong interpersonal skills, communication, collaboration, and conflict resolution Ability to work independently, exercising discretion and independent judgment Ability to work in collaboration with other disciplines within the organization Ability to travel to travel to the patients home Must be able to physically access any home in any area of Gulfside Home Health service area, i.e., navigate stairs, and narrow spaces, tolerate heat and lack of air conditioning if necessary.
