What are the responsibilities and job description for the CNA position at Home Caregivers Partnership LLC?
QUALIFICATIONS
1. Has completed a nurse aide training and competency evaluation program approved by the state, and is currently listed in good standing on the state nurse aide registry.
2. Has furnished services for compensation within the last twenty-four (24) months. A lapse greater than 24 months requires the completion of another aide training program and certification requirements per state regulations.
3. Have a sympathetic attitude toward the care of the sick and elderly.
4. Ability to carry out directions, read and write.
5. Ability to effectively communicate to demonstrate competency in the provision of care and services safely and effectively and treat patients with dignity and respect to person and property.
6. Maturity and ability to deal effectively with the demands of the job.
7. Must be 18 years of age or older.
RESPONSIBILITIES
1. Provides services that are, ordered by the physician, included in the Home Health Aide plan of care, is permitted to be performed under state law and is consistent with the Home Health Aide training.
2. Performs hands-on personal care.
3. Performs simple procedures as an extension of the therapy or nursing services as assigned.
4. Assistance in ambulation or exercises as assigned.
5. Assists with medications that are ordinarily self-administered as assigned.
6. To follow basic infection control procedures.
7. Understands and adheres to established Agency policies and procedures.
8. Completes appropriate visit records in a timely manner as per Agency policy.
9. Reports changes in the patients’ condition and needs to the appropriate personnel.
10. Ability to recognize and report changes in skin condition.
11. Performs household services essential to health care in the home as assigned.
12. Must receive at least 12 hours of in-service training during each 12-month period.
13. Attends and participates in staff meetings and patient care conferences as part of the IDT team.
14. Follows Agency policy for cleaning equipment between patient uses.
15. Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
