Responsibilities
As a Physical Therapist (PT) at Family Home Health Services you will:
- Provide therapy services for each patient based on their plan of care, within your scope of practice, in the patient’s home.
- Complete comprehensive assessments incorporating the OASIS data set, as required by Medicare, utilizing OASIS information to assist in the development of the plan of care.
- Assists the physician in evaluating the patient’s level of function and helps develop and revise the plan of care.
- Evaluate patient for appropriateness of admission and verify the patient meets homebound status.
- Educate the patient and family members on treatment plan, goals and expected outcomes.
- Prepare and submit clinical and progress notes, reflecting activities, findings and patient’s reaction to treatment and changes to patient’s condition in a timely manner following visit or communication.
- Attend virtual staff meetings and participates in patient case conferences/coordination of care communication.
- Other duties as assigned.
Qualifications
Requirements:
- Graduate of an accredited Physical Therapy program.
- Licensed as a Physical Therapist in Florida.
- At least one year of experience treating adult patients, preferably geriatric patients.
- CPR certification.
