HonorHealth is a non-profit, local community healthcare system serving an area of 1.6 million people in the greater Phoenix area. Responsibilities
Job Summary
Provides expertise in clinical pharmacy activities relating to various clinical programs for Innovation Care Partners (ICP) in order to ensure optimal patient outcomes. Facilitates the development, implementation, and maintenance of programs that promote desirable patient outcomes and high-value care through the use of safe and cost-effective drug therapies. Serves as the clinical pharmacy expert with analyses of medications for quality of care, utilization, efficacy, and costs for population management.
- Quality Improvement
This role is responsible for working with the CMO and other ICP leaders in executing and scaling ICP’s strategy to enhance success in pharmacy initiatives such as medication adherence, medication management, and high-value medication prescribing to drive performance in value-based care arrangements.
Strong cross-functional collaborator that will work with the Population Health team, Informatics, and Clinical Team as directed by the CMO to improve patient safety and patient outcomes.
Evaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes by working directly with providers and other health care team members and patients.
Contributes to performance improvement, clinical effectiveness, and drug use evaluation through development of pharmacy related initiatives.
May provide medication reviews and reconciliation for complex patients, providing recommendations when necessary for improved patient safety, outcomes and high-value care. - Education
Suggests appropriate and cost-effective therapeutics, and assists with evidence-based medication related guideline development to ICP Committees and Work Groups as needed.
Participates in staff educational development encouraging team members to further their learning in pharmaceutical care to contribute to patient care and safety. Provides educational information to providers, care management, and other clinical teams as needed or assigned. - Liaison
Serves as a resource to and collaborates with the ambulatory pharmacy team at HonorHealth to improve access to services and captures of quality measures.
Participates in the HonorHealth Employee Plan pharmacy meetings and payer quality meetings that are relevant to this role’s area of expertise.
Liaison to providers and care management teams by communicating pertinent information and facilitating communications, guidance, and compliance with medications. - other duties as assigned.
Qualifications
Education
Pharm.D. Graduate of an ACPE-accredited School of Pharmacy with a minimum of Doctor of Pharmacy degree or equivalent experience Required
Experience
1 year Hospital Pharmacy Residency or 3 years of Clinical Pharmacy experience in a hospital and/or ambulatory setting. Required
Licenses and Certifications
Registered Pharmacist (RPh) Licensed to practice pharmacy in the state of Arizona. Required
