Wages & Benefits
$23.00 - $25.00 per hour; PTO accrues for all hours worked; paid holidays for the year plus 2 floating holidays; affordable health, dental, and vision insurance; employer paid term life and disability coverage; retirement plan with immediate vesting and matching contribution.
Schedule
This position will be primarily in schools doing outreach typically with one and a half days per week in an office setting and working until 7 p.m. two days per week, generally flexing hours to keep to 40 hours per week. This position must have reliable transportation as the ability to meet with youth in the community is required.
A Typical Day in the Life…
A typical day as HOPE 4 Youth’s Youth Navigator provides an opportunity to deliver professional, compassionate, and quality case management and assistance to youth experiencing homelessness within the Anoka-Hennepin and neighboring school districts. Responsibilities will include the following:
- Providing case management to youth in alignment with their goals towards sustainable housing.
- Completing intakes and assessments for new youth participants.
- Providing youth with referrals to community resources and services.
- Documenting contact, enrollment, services, referrals, case management hours, and case notes in data management system.
- Completing coordinated entry intakes in HMIS to place youth in priority pool.
- Creating a safe and supportive environment for all youth, especially LGBTQI youth and those from racially and culturally diverse populations.
- Providing guidance and support to school personnel, social workers, and homeless liaisons.
- Maintaining safety policies and procedures within outreach locations.
- Attending and representing HOPE 4 Youth at various community meetings, such as: Housing Response Team Meetings, Heading Home Anoka Housing Collaborative, SMAC Committee meetings, Anoka Shelter Team meetings and others as requested.
- Assisting with the facilitation of and transportation for weekly support groups.
Who You Are
- You have at least 3 years of relevant professional or lived experience in the field of case management with at-risk homeless youth ages 16- 24.
- Current training through Streetworks is preferred, but not required.
- You demonstrate the ability to work effectively in a diverse, mission-driven agency committed to the principles of diversity, equity, and inclusion with respect to race, ethnicity, gender identity, sexual-orientation, socio-economic status, nationality, and religion.
- You have a firm grasp and understanding of effective crisis intervention and programming that is trauma informed, strengths-based, client-centered, and follows a harm reduction philosophy.
- You are proficient in MS Word, Excel, and are able to produce letters, emails, and reports of professional quality.
- It is preferable if you have experience with using data and databases.
- You have the ability to communicate in writing and verbally in English, and a secondary language is preferred.
- You have a valid driver’s license, access to reliable transportation, and the ability to meet H4Y’s driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
