HopeWest PACE (Program of All-Inclusive Care for the Elderly) started in October 2021 and continues to grow. Through this innovative new program called PACE, we balance the medical, emotional, physical, and social needs with care that allows each participant the opportunity to stay safely in the place they call home.
We are looking for an experienced Registered Nurse (RN) leader to become our Home Care Manager for PACE. This position provides the opportunity to provide in-home nursing care, clinical oversight, and training for the CNAs, PCAs, and Home Care RNs. The Home Care Manager will promote and maintain the health of participants in the community setting through teaching, counseling and preventative or skilled services. This role works alongside the 11-member interdisciplinary team to assess the participants as they come on service with us, and again periodically to re-evaluate the individual and family needs. In collaboration with the Sr. Director of Clinical Services, the Home Care Manager will create and implement departmental policies and procedures that align with the goals of HopeWest and the PACE program.
What You Bring:
· Registered Nurse with current Colorado licensure
· At least 3 years’ experience, in a home care setting preferred
· Minimum of 1 year management experience
· Valid Colorado drivers license, adequate automobile insurance and reliable transportation
· Ability to work collaboratively and communicate effectively

· Monday-Friday, 8am-5pm schedule with on-call rotation required
