We have openings in our community setting for both day & night shifts. Our Community Team sees patients in their own residentials homes or other care facilities. Licensed Practical Nurses LPN focus on intensive medical end-of-life care for patients. Our approach to care includes comprehensive physical, emotional, social, and spiritual support.
You will be part of an interdisciplinary care team of Nurses, social workers, chaplains, physicians, and certified nursing assistants all working together to improve people’s lives when they need help the most. You will be one of those people patients and families call “angels” -- offering miracles of human caring, compassion, strength, and courage.

This position provides professional nursing care within the parameters of the established care plan, under the supervision of an RN and within the scope of practice of the Licensed Practical Nurse (LPN) as defined by the Colorado State Board of Nursing.
Requirements:
- Current Colorado Licensure as a Licensed Practical Nurse
- Graduated from an accredited school of nursing
- Hospice experience or two years of acute or long-term experience
Pay and Benefits:
- Pay: $25.00 - $27.00 Hiring Range Base Wage
- Additional pay for evenings, nights and weekends and bilingual fluency
