The Care Managers main purpose is to ensure their client caseload is provided with the necessary services they need, in a coordinated and effective manner. Responsible for the caring and compassionate delivery of case management, advocacy and other supportive services to ensure appropriate coordination of services for clients.
Qualifications Required:
- HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience with at least 1 year case management experience
- Ability to create plans and summaries based on client needs
- Certification in first aid, CPR and CPI (training will be offered if do not possess upon hire).
- Must be at least 21 years of age or older.
- Possess valid drivers license with acceptable driving record and reliable transportation.
- Prior experience in the behavioral health setting is strongly preferred.
- Demonstrated organizational abilities to maintain communication, coordination, and connections with clinical team participants.
- Willingness to work evenings as needed.
- Ability to effectively use CPI and behavior management techniques in crisis situations.
- Desire and capacity to maintain time-management and organization while working in fast-paced, hands-on work environment.
- Must obtain and maintain a fingerprint clearance card and pass a pre-employment drug and background check.
