The Support Coach is responsible for ensuring the physical and emotional health and safety of each resident at the home where they reside.
The Support Coach helps foster the independence of the resident, supports them in achieving their goals, and facilitates opportunities for them to live a fulfilling and meaningful life.
In this position you will :
- Assist residents with activities of daily living, offering verbal reminders or physical assistance as needed
- Assist resident(s) with the maintenance of their home and preparation of meals in keeping with specific diets
- Implement a program that includes goals and objectives as established by the person's team in the Individuals Service Plan
- Learn strengths and preferences of the individual(s)
- Administer medication as directed by the Manager in accordance with Physician's Orders and DDS Medication Administration guidelines
- Be accepting of people's differences (e.g. abilities, styles, personalities, etc.) and adjust own approach to accommodate others
- Communicate in a way that is accurate, timely, positive, person-centered and respectful of the person's privacy
- Complete all required documentation daily and participate in regular staff development sessions
- Keep manager(s) informed of incidents, behaviors, comments, etc., regarding resident(s) that are unusual, problematic or significant
- Intervene in emergency situations (e.g., self-injury, medical aggression) and according to an individual's specific behavior management program utilizing training received from Horizons
Benefits :
Competitive hourly wage starting at $18.00 to $20.00 per hour

Requirements :
- The Support Coach is at least 21 years of age
- Must possess a high school diploma or equivalent
- Must possess a valid Connecticut driver's license, acceptable driving record, and reliable transportation
- Must possess at least two years minimum experience working with people or an approved equivalent
- Must be able to pass a criminal background & license check performed by Horizons
- Physical Requirements - Ability to lift at least 50 pounds; ability to respond to critical incidents that may require protective holds and escorts;
ability to assist individuals to enter / exit buildings, work sites, vans / vehicles, etc; ability to perform CPR and First Aid
