We are seeking dedicated and skilled Registered Nurses for PRN shifts to join our South team which services counties South of Charlottesville, Virginia. As a Registered Nurse in our hospice program, you will play a vital role in ensuring the comfort, dignity, and well-being of our patients.
Responsibilities:
- Perform nursing evaluation visit, develop plan of care in conjunction with physician’s orders and in coordination with disciplinary team members, based on patient’s needs.
- Assess and re-evaluate patient needs, consulting with physicians, reporting changes, and communicating with others when a change in the Plan of Care is necessitated to ensure optimal comfort and quality of life.
- Provides education to patients, families, and caregivers about disease progression, symptom management, and end-of-life care options.
- Coordinate care with other healthcare professionals, including physicians, social workers, chaplains, and CNAs, to ensure a solid team approach to patient care.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions by practicing appropriate documentation at the bedside using the Electronic Medical Record (EMR) and prepares clinical progress notes on nursing services delivered in a timely manner.
- Participate in interdisciplinary team meetings, providing valuable input and contributing to the overall care plan.
- Stay current with best practices, research, and developments in hospice care, seeking continuous professional development opportunities.

Qualifications:
- Current and unrestricted Registered Nurse (RN) license in the state of Virginia.
- Minimum of 2 years of nursing experience preferred, preferably in hospice, palliative care, or a related field.
- Knowledge of hospice regulations, practices, and standards of care with a demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management.
- Excellent clinical assessment and critical thinking skills, emotionally mature with the ability to cope with family stressors and tolerance of individual lifestyles.
- Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
- Compassionate and empathetic nature, with a genuine desire to provide comfort and support during challenging times.
- Ability to work independently and as part of a team, collaborating effectively with interdisciplinary professionals.
- Highly organized, self-motivated with the ability to articulate information with a diverse population, and proficient in using electronic medical records (EMR) and other healthcare software.
