What are the responsibilities and job description for the On Call RN, PRN position at Hospice of the Upstate?
FUNCTION
Provides nursing care to Hospice of the Upstate patients in accordance with Medicare Conditions of Participation and organizational policies following medical directives and Hospice plan of care. This position will be managing the weekend shift including Triage, 24 hour symptom management, Admissions, Visits & Deaths.
SUPERVISION RECEIVED
Works under minimal supervision while ensuring quality health care. Reports to the Home Care Patient Care Coordinator.
QUALFICATIONS
1. Two years experience in hospice care preferred.
2. Must have a current South Carolina license as a Registered Nurse.
3. Must have a valid driver’s license and proof of automobile insurance at required levels.
4. Must have reliable transportation.
5. Demonstrate the ability to work well and professionally under stress with flexibility and independence.
6. Demonstrate the ability to manage workload and schedule effectively and efficiently.
7. Must be empathetic and have the ability to relate well to patients, families, and members of the Hospice of the Upstate care team.
8. Understands and embraces the hospice philosophy.
RESPONSIBILITIES

- Responds promptly to patient/family needs after hours in a manner that promotes the quality of care expected by Hospice of the Upstate.
- Documents routine visits, follow-up visits, on-call visits, and phone calls within 24 hours of the event.
- Documents visits and patient/family contact in appropriate note with visit type.
- Provides education, training, and support to the hospice patient/family as directed by the patient’s plan of care and as needed-understanding that patient/family educational needs will change as the patient’s disease progresses.
- Communicates patient needs and changes to Hospice Medical Director, attending physician, and contracting agencies that provide services.
- Develops, implements, and updates team care plans reflective of patient/family wishes and appropriate level of care.
- Ensures all guidelines are met for changes in level of care and care location.
- Communicates and collaborates with Rainey Hospice House staff regarding transfers of home care patients.
- Ensures continuity of care if patient is hospitalized.
- Ensures medication profile remains current.
- Triages calls in a manner that acknowledges emergent needs.
- Reviews the clinical records and completes a physical assessment of the patient
- Listens to the concerns and needs of the patient and family
- Effectively communicates specifically how Hospice of the Upstate will address the expressed needs and concerns.
- Determines hospice eligibility with attending physician and Hospice Medical Director.
- Responsible for any information communicated in staff meetings.
- Practices within the guidelines of the appropriate Boards of Nursing.
- Meets competency requirements for discipline and role.
- Assists other departments with a spirit of teamwork and cooperativeness
- Ensures the protection of patient and employee information by following the HIPAA-HITECH guidelines.
- Professionally represents the organization with credibility and dignity and in the spirit of excellence and trust.
- Exhibits flexibility, organization and works effectively and professionally under stress.
- Demonstrates and communicates the mission, ethics, and goals of the organization as the face of Hospice.
24. Assists in Rainey Hospice House as needed/requested
25. Assumes other duties as necessary.
TOOLS AND EQUIPMENT USED
Standard medical equipment: Lap top computer, phone, BP cuff, and stethoscope.
PHYSICAL DEMANDS
May be required to climb steps, stoop, bend or lift up to 35 pounds. Must be able to communicate compassionately with patients, families and the general public.
Pay: $33.00 - $37.00 per hour
