Our RN Case Managers serve hospice patients wherever they call home. This role provides much needed, compassionate care for our patients, meeting them where they are, while also allowing for the autonomy necessary to maintain the work/life balance we all strive for.

Team Description: Our HomeCare RN Case Managers provide nursing services and education to individuals in their homes. Our Case Managers coordinate plan of care by working with the interdisciplinary team to anticipate and meet patient needs and specific plan of care goals. Our interdisciplinary team includes the RN, Social Worker, Chaplain, Nursing Assistant, Volunteers, Nurse Practitioner and Team Manager – all working together to ensure the needs of our patients are met.
Qualifications
A Diploma or Associates Degree in nursing is required; Bachelor’s Degree preferred.
A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
Current CPR required; CHPN preferred.
Minimum 2 years related experience; Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
Experience in direct care of patients with life limiting illnesses required.
Computer proficiency is required.
Salary : $63,600 - $80,500
