What are the responsibilities and job description for the RN Case Manager-Hospice position at Hospice Promise?
The RN/Case Manager is responsible for administering all skilled nursing care for clients of all ages in their place of residence. They will coordinate the care with the interdisciplinary team, patient/family and referring agency, and assumes the responsibility for coordination of care. The RN/ CM plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
