Compensation Range: $63,000 - $68,000 annually commensurate with experience
Overview:
The Adult Day Health Care (ADHC) Program at Housing Works is a unique community-based health care program that provides comprehensive nursing, social work, nutrition, mental health and substance use services to individuals living with chronic illnesses including HIV, behavioral health conditions, diabetes, hypertension and others. Housing Works is seeking a Clinical Supervisor to serve as lead Supervisor, responsible for oversight of the Clinical services at our ADHC location Downtown Brooklyn. This is a unique opportunity for community for an individual with a strong clinical background /experience and management interest to work in a collaborative, outcome focused setting. With the support of the Clinical Director and an interdisciplinary team of professionals, the Clinical Supervisor will help achieve improved health outcomes for our client population. The ideal candidate will be familiar with population health model and comfortable using data to guide practice and supervision. Responsibilities:
- In conjunction with the Program Director , plans and administers all of the social work and case management services for the ADHC.
- Administers bi-annually satisfaction surveys to obtain feedback about the current service; develops a strategic plan for the Clinicians to maximize and improve the quality of case management within the program; including but not limited supportive counseling and group work and crisis intervention services. Monitors and oversees all case management services including case distribution and review of case records.
- Completes ADHC monthly reports and complies with quarterly and annual contracting reporting as denoted by the regulatory agencies, in coordination with the development department.
- Reviews and modifies initial and annual psychosocial and substance use assessment and reassessment forms and educates other staff members about record-keeping policies that ensure comprehensive client care in order to meet the regulatory agencies’ guidelines.
- Participates in the peer review and QA/QI committee and thereby ensures, by regular monitoring, that client chart documentation in accordance with all state and federal laws, rules, and regulations.
- In coordination with the assigned RN, leads the interdisciplinary care planning sessions with role of ensuring that goals and interventions are truly comprehensive and inter-disciplinary.
- Develops tracking mechanisms to monitor clients’ discharges and closely oversees the provision of services for enrolled clients.
- Ensures the integrity of the ADHC program by maintaining confidentiality, ethics and professional conduct throughout the program.
- Facilitates psychosocial support groups and provides individual supportive counseling to ADHC clients when needed.
- Co-leads clinical group interventions and lend support to the clients and the team
- Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS
- Monitor program compliance and quality assurance
- Participate in agency wide continuous quality improvement activities aimed at improving health outcomes for program participants
- Assist with the development of effective interventions and treatment plans
- Provide direct clinical and administrative oversight of program staff.
- Other duties as assigned
- Master’s degree in social work (LMSW)
- Experience or sensitivity to working individuals or families who have issues of mental illness, substance use, homelessness or HIV/AIDS
- Previous supervisory experience preferred but not required
- Strong leadership ability
- Ability to use data to inform clinical and administrative decision making
- Ability to apply population health and Continuous Quality Improvement principles to work
