Blended Case Manager

```Duties```
- Provide case management services to individuals in need, including assessment, planning, and coordination of services
- Conduct comprehensive assessments to determine clients' needs and develop individualized service plans
- Collaborate with clients to set goals and monitor progress towards achieving them
- Coordinate and advocate for necessary services, such as healthcare, housing, employment, and education
- Provide crisis intervention and support during times of emotional distress or emergency situations
- Utilize motivational interviewing techniques to engage clients in the treatment process
- Assist clients in accessing community resources and support networks
- Maintain accurate and up-to-date case records and documentation

```Requirements```
- Bachelor's degree in social work or related field required; Master's degree preferred
- Previous experience in behavioral health or social work strongly preferred
- Knowledge of crisis management techniques and intervention strategies
- Familiarity with Medicare regulations and requirements
- Ability to effectively supervise and provide guidance to support staff
- Strong communication and interpersonal skills
- Ability to work collaboratively with a diverse population of clients and professionals
- Knowledge of addiction counseling principles and techniques is a plus
- Experience in workforce development or childcare services is a plus
