The Home Health Aide 2, is responsible for performing assigned functions of personal care to the patient in their residence under the direction, instruction, and supervision of a registered nurse (RN). Such care must be provided in accordance with current Federal, State, and local standards, guidelines, and regulations that govern the agency, and as may be required by the Clinical Manager, Director of Nursing, or Administrator to ensure that the highest quality care is always maintained.

               

  • Performs all personal care activities which are permitted to be performed as contained in a written assignment by a licensed health professional (Aide Care Plan)
  • Reviews safety risks within the patient’s home and take appropriate action. 
  • Notifies supervisor on risks and actions, documentation actions are taken.
  • Protects patient from injury while performing activities.
  • Uses appropriate techniques in performing personal hygiene and grooming tasks that include:
    •     Bed bath
    •     Sponge, tub, and shower bath
    •     Hair shampooing in sink, tub, and bed
    •     Nail and skin care
    •    Oral hygiene
    •    Toileting and elimination
  • Recognizes emergencies and has the knowledge of instituting emergency procedures within their scope if the situation arises while providing patient care.
  • Communicates effectively through the ability to read, write and verbally report clinical information to patients, representatives, and caregivers, as well as other HHA staff.
  • Reports to the appropriate supervisor changes in the patient’s condition including changes in appearance, behavior, and other elements of body functioning.
  • Recognize and report changes in skin condition, including pressure ulcers to the Director of Nursing/Administrator or other members of the health care team.
  • Provides proper body positioning for patients.
  • Assists with restorative exercises to include a normal range of motion (ROM) and/or other specific exercises and treatments assigned by a registered nurse, physical therapist, or occupational therapist.
  • Uses safe transfer and ambulation techniques including the use of assistive devices when assisting patients.
  • Measures and records temperature, pulse, and respiration
  • Maintains a clean and healthy patient environment.
  • Measures and records fluid intake and output when indicated in the care plan.
  • Assistance in administering medications ordinarily self-administered.

Required Qualifications

  • Clear and Active Certified Nursing Assistant and/or HHA (75 Hours) certification to practice in the state of Florida.
  • Updated annual 12 hours of In-Service Training
  • CPR Certification
  • This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
  • Excellent customer service

  • Strong written and verbal communication skills

Preferred Qualifications

  • High School Diploma or GED
  • Knowledge of primary care medicine requirements (state specific).
  • Working knowledge of clinical operations
  • 1-year experience working with a Home Health agency
  • Electronic Medical Record (EMR) experience
  • Previous Medical Assistant experience
  • Ability to type 25-30 words per minutes accurately
