ESSENTIAL JOB FUNCTIONS

- Function as integral ICL Hope Center team member engaged in comprehensive coordination and communication with ICL Hope Center staff, Entitlements Department, and ICL treatment, housing and care coordination programs.
- Respond to internal referrals, referrals from external providers, and people seeking services through intranet, email, and by phone. Ensure all referral information is accurately documented in referral management system
- Conduct whole health brief screening for housing, care coordination and treatment services
- Schedule housing interviews and intake appointments
- Ensure match between individual’s needs and preferences and program eligibility criteria
- Review referral material for completeness. Call referral sources to gather more referral information if needed.
- Maintain accurate information on ICL program types, admission criteria, and program services to facilitate highest quality care matching.
- Support referral and intake workflows to ensure effective and efficient admission process.
- Collaborate with external service providers and entities to ensure coordination of care and making referrals to outside providers as necessary.
- Maintain housing waitlist, checking on status of individuals on the waitlist and communicating with referring providers
- Maintain contact with referring providers providing feedback regarding vacancies, planned resident movement, acceptance status, etc.
- Ensure all information is accurately documented in the electronic record in the required timeframe.
- Act as ICL whole health ambassador including value of equity and reducing health disparities
- May be assigned other tasks and duties reasonably related to the job responsibilities.
MINIMUM QUALIFICATIONS
- Master’s Degree in Social Work or other relevant field or
- Bachelor’s Degree with at least 2 years of experience working with individuals with mental illness, substance use disorder and/or experiencing homelessness, or with lived experience of mental health or substance use recovery
- Experience with NYS/NYC eligibility criteria, and funding sources
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
- Highly organized and detail-oriented
- Excellent written and verbal communication skills
- Positive attitude and excellent customer service skills
- Ability to engage and interact well with a wide range of persons at organizationally different levels
- Knowledge of federal, state, and local regulations pertaining to the admission, treatment and discharge of persons with mental illness and co-occurring disorders
- Proficient in Excel
- Knowledge and applied understanding of confidentiality
- Knowledge of various types of community mental health programs and their admission and discharge requirements
- Ability to collaborate with and cultivate relationships across ICL divisions and with external partners
- Ability to prioritize and follow through on multiple tasks simultaneously
