Job Title: Community Health Worker (Bilingual, Spanish-English)
Reports to: Clinic Nurse Practitioner
Summary: A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The community health worker serves as an integral part of the IFM health team (Physician-MD, Nurse Practitioner-NP, Medical Volunteers, Community Health Worker-CHW). In this position, the CHW would join a team of NPs and CHWs providing care in the Bridgeton-St. Ann area.
Job Description Responsibilities:
- Serve as a health navigator, working one-on-one with patients who screen positive using a Social Determinant of Health (SDoH) screening tool
- Upon referral from the MD/NP, meet with patients to provide health education to promote better understanding of disease processes, provider instructions, and treatment plans
- Strongly encourage patient compliance with scheduled appointments and treatment plans
- Assist with the flow of the clinic, including translation for Spanish speaking patients and translating clinic materials from English to Spanish
- Act as patient advocate for the rights of culturally diverse clients and their families
- Assist patients in health insurance (Medicaid or ACA) and/or prescription assistance enrollment
- Assist patients in connecting with community resources
- Patients screening positive/requesting assistance, will have their needs addressed at the time of their appointment or they will be contacted within two working days via phone by a CHW
- Assist with clinical care duties including: taking calls, scheduling appointments, patient check in, and obtaining vital signs
- Document interventions and encounters in Practice Fusion Medical Record
- Track and report patient encounters and complete monthly reports
- Maintain appropriate confidentiality of information, in compliance with HIPAA regulations
- Actively participate in required training and professional development activities
- Actively participate in key meetings; internal (weekly-local and monthly-group) and external/community (Unite Us, IHN and other organizations, as appropriate)
- Develop and maintain collaborative referral relationships with key community agencies. Include progress in monthly report
- Identifies strengths and barriers of current referral and management systems. Assertively works to build on strengths and resolve barriers to promote positive health outcomes
Qualifications:
Education: High School Diploma required. CHW state certification or willing to become certified required. Bachelor's degree preferred
Experience: One or more years of health care or social services experience preferred. Experience in a medical office or as a medical assistant is a plus.
Other skills & knowledge: Strong relationship building and collaboration skills. Excellent communication and facilitation skills. Fluent in Spanish and English and willing to become medical translator certified. Ability to work independently and function as part of a team, have good time management skills, and attention to detail. Compassion for the poor and underserved communities. Proficient in MS Office - Word and Excel. Must have reliable transportation/personal vehicle. Ability to communicate with people with various levels of medical knowledge, in both written and verbal forms. Knowledge of regulations related to Medicare, Medicaid, and commercial insurance. Knowledge of maintaining supplies and equipment for the medical setting. Knowledge of medical terminology, ICD and CPT coding, and HIPAA regulations.
