OPENING FOR FULL TIME BOARD CERTIFIED NURSE PRACTITIONER IN NEW BRITAIN, CT
Growing ‘NP Team’ company will offer the opportunity to manage acute and chronic medical conditions in the long term care, transitional care, nursing home facilities. Other opportunities may become available in near future.
The healthcare delivery model of InHouse Care LLC allows the APRN to exercise their clinical skills developed through education, experience, and training. You will become a critical member of the facilities you cover.
Job duties will include collaboration with other providers and facility staff to formulate treatment plans of acute and chronic conditions. The NP will provide direct (hands-on) care in nursing homes/skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.
Responsibilities:
- Diagnose and treat acute, episodic or chronic illnesses
- Prescribe all necessary medications and treatments
- Perform comprehensive physical examinations of patients
- Develop and implement patient management policies and procedures
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with collaborating physician or specialist regarding patient care
Geriatric or long term care experience is preferred.

Completion of master’s degree or comparable graduate-level training at an accredited nursing school/program Active with unrestricted APRN licensure by the Connecticut Board of Nursing. Active DEA registration.
