This opportunity will give you a chance to grow relationships with your member caseload as you coordinate services for them and perform in-home visits every 6 months.
This is a Monday-Friday role, with the core business hours of 8am-4:30pm. We provide unique flexibility that allows colleagues to do things that are important to them. Many of our colleagues drop off and pick up their children from school, step out for medical appointments easily, or complete household tasks at home all while meeting required work obligations and not utilizing PTO!

What are the primary duties of this RN position?
- Partner with a social services care manager also referred to as a Community Resource Coordinator, and collaborate on an ongoing basis regarding the member and their plan.
- Perform face-to-face health-related assessments for members upon enrollment and at least every six (6) months thereafter in the member’s setting.
- Participate in the development and ongoing review and coordination of the member’s plan of care.
- Take the lead on all health issues for the member.
- Contact and coordinate with acute and primary care providers.
- Identify and coordinate clinical and functional supports needed for the member’s health and safety.
- Educate on and promote prevention and wellness and mitigate risk when assessed.
- Document activity and complete paperwork as required.
- Traveling will be required and eligible for mileage reimbursement.
