The Care Manager Delivers top quality care management services to transitional care patient population through working collaboratively with the multidisciplinary team. Advocates, guides, and intervenes on behalf of patients, families, and/or caregivers. Engages identified patients in appropriate care management programs and provides patient centered interventions. Strives to positively influence the overall patient health outcomes as measured by clinical quality and utilization measures. Supports the achievement of system-wide population health goals within the framework of continuous improvement methodologies.
- Associate's Degree in Nursing required. New ASN RNs hired after January 1, 2013, will be required to complete their BSN within 5 years of hire date.
- Requires current state of Indiana license as a Registered Nurse.
- Certification in Care Management preferred.
- Strong professional level knowledge of comprehensive clinical assessment skills preferred.
- Knowledge of Medicare/Medicaid, insurance and regulatory guidelines is preferred.
- 3-5 years of relevant experience required.
