Job Description:
I. Job Summary: Employee administers appropriate nursing care residents in the skilled nursing facility under the supervision/direction of a registered nurse or physician. Performs a variety of direct and indirect patient care duties and activities. Supervises CNA staff.
Employees in this position require occasional supervision while working toward a definite objective requiring use of a wide range of procedures: plan, arrange, and/or determine specific procedures or equipment needs required to meet assigned objective: solve non-routine problems, referring only unusual matters to supervisor.
II. Minimum Qualifications:
A. Education
1. High school graduate or equivalent.
2. Successful completion of an accredited LVN/LPN program.
B. Experience
Level I Entry level. New graduate, or less than one (1) year experience, or less than six (6) months acute or skilled nursing experience. Mastery of basic nursing skills.
Level II Mastery of basic nursing skills. Minimum of one (1) year nursing experience and six (6) months acute or skilled nursing experience. Demonstrates good supervisory skills with CNA’s, maintains good attendance record, seeks RN instruction when needed. Demonstrates good time management.
Level III Meets Level II criteria, plus proves capable of making appropriate independent decisions. Demonstrates good judgement, attends 75% of department meetings and in-services, and demonstrates a desire to learn. Demonstrates understanding of State and Federal regulations pertaining to skilled nursing facilities.
C. Licensure / Certifications / Registrations
1. Current valid California LVN license or Interim Permit.
2. Valid Basic Life Support/Healthcare Provider certification.
Level II
1. Meets Level I criteria.
2. Infection Prevention Certification preferred.
3. MDS Certification preferred.
Level III
1. Requires IV Certification, or Director Staff Development certification.
2. Infection Prevention Certification required within 1 year.
3. MDS Certification required within 1 year.
D. Essential Skills
1. Understanding of basic medical terminology.
2. Knowledge of basic nursing procedures.
3. Ability to perform various nursing procedures required for good patient care.
4. Ability to understand and carry out written, oral, and/or graphic instructions.
5. Ability to accurately perform intermediate math, including addition, subtraction multiplication, and division.
6. Ability to write and prepare clear and concise reports.
7. Ability to speak effectively on a one on one and small group situation to patients, visitors, and other employees.
8. Ability to comply with procedures for infection control and asepsis.
9. Knowledge of hospital equipment, supplies, and instruments; and the ability to properly use them.
10. Ability to provide sympathetic nursing care.
11. Understanding of and ability to use proper body mechanics to prevent injury to self.
12. Ability to keep and document accurate patient records.
13. Establish and maintain effective work relationships with those contacted in the performance of your duties.
14. Understanding of the principles and practices of licensed vocational nursing.
15. Understanding of pharmaceuticals prescribed for the elderly, and has knowledge of actions of medications and their side effects.
16. Ability to organize and manage multiple tasks.
