  1. SUMMARY: The Program Supervisor will provide day-to-day oversight of assigned areas including Residential Services, Supported Living Services (SLS), Children's Extensive Services (CES), work crews, and Community Support Services (day program). Responsibilities include staff and individual supervision, program development, coordination and delivery of program services for individuals in a variety of day habilitation, residential, and community-based settings. This position is also responsible for maintaining program files and assisting in achieving program goals as needed to maintain program accreditation. This position is responsible for the administration of necessary physical and emotional care to promote growth and development of individuals in service in a person-centered environment. The Program Supervisor must exercise discretion and independent judgment in the performance of job duties.
  1. ESSENTIAL DUTIES and MINIMUM QUALIFICATIONS:
  1. Oversee day-to-day operations of programs to include but not limited to, providing appropriate training, scheduling, and supervision for staff and individuals and ensuring that appropriate program staff performs teaching and supervision of individuals at assigned sites and community-based settings.
  2. Supervise staff by using consistent application of personnel policies and procedures. This includes but is not limited to the following: interviewing; hiring for program/site vacancies; providing site/job specific training for new hires and veteran staff; providing feedback on staff performance; writing and delivering performance evaluations and disciplinary action, ensuring adequate staffing at all times, etc.
  3. Obtain, analyze, and synthesize case information on individuals in order to develop and implement Individual Program Plans and ISSPs. This may include but is not limited to attending planning meetings for individuals and providing formal and informal assessments of individuals and summarizing functional skill levels and behaviors for the Individual Plan.
  4. Maintain completeness of program files and records including: counting and ordering medications; organizing and maintaining updated program files; balancing individuals' personal accounts; summarizing training plans; following procedures as required to maintain program accreditation; writing ISSPs; monitoring ongoing tracking of program plans; utilizing community resources in designing and coordinating community integration programs for assigned individuals; and assisting in developing and achieving program goals.
  5. Maintain assigned sites in compliance with the standards set forth by the agency, Colorado Department of Human Services, Colorado Department of Public Health and Environment, Health Care Policy and Financing, and other state, local, and county regulatory agencies.
  6. Pass the Medication Administration test with no more than two attempts and administer medication to individuals according to physician orders and using appropriate administration techniques as outlined in the Medication Administration course.
  7. Must be able to perform Lifting and Transferring techniques as trained and pass performance testing. The employee may occasionally lift and/or push and pull up to 50 lbs. Lifting may require floor to waist, waist to shoulder, or shoulder to overhead movement.
  8. Driving of agency vehicles as necessary for transportation of individuals and agency business as assigned.

  • Able to regularly sit, walk, stand, climb up and down stairs, physically restrain individuals pursuant to training (when necessary) and sit at a computer for long periods of time and repeat the same hand, arm, or finger motion many times (typing, data entry).

  • Operate a personal computer and related software.
  • Tolerate various levels of stress and meet deadlines with severe time constraints.
  • Demonstrate specific vision abilities which include close, distance, and depth perception and are able to read handwritten or typed material, and able to adjust focus.
  • Work and concentrate in both noisy and quiet environments.
  • Squat, stoop, kneel, and reach above the head. Forward motion will occasionally be required.
  • Work inside the building a majority of the time.

ADDITIONAL DUTIES AND RESPONSIBILITIES:

  1. Provide documentation within established deadlines as required by the agency and other regulatory agencies including, but not limited to, progress/training notes, attendance forms, incident reports, and other documentation as assigned.
  2. Maintain certifications for First Aid, CPR, crisis prevention and other courses as required.
  3. Assist with the development of behavior programs and provide necessary tracking, baseline data, and follow up. This may include monitoring and tracking plans and progress as required.
  4. Plan, create, and implement a schedule of interactive and educational activities for individuals to increase engagement during programming hours. Individuals in service should assist in creating the schedule of activities.
  5. Seek out and involve individuals in community employment and volunteer activities.
  6. Oversee community-based and non-integrated activities and employment to ensure consistent implementation and alignment with the agency's mission and Strategic Plan.
  7. Develop and maintain relationships with community resources and contacts for community integration, employment opportunities, and contract labor.
  8. Work with Job Coach to develop and maintain community job placements for individuals.
  9. Effectively oversee work store operations by ensuring that staff and individuals are properly trained and consistently working to increase sales and to create meaningful work.
  10. Conduct regular program site meetings and document minutes of these meetings.
  11. Be available via cellphone on a scheduled basis and also on as-needed or on-call basis to address issues involving individuals in service and general program l issues and/or emergencies, as needed.
  12. Monitor sites and ensure the upkeep and satisfactory environment of assigned sites.
  13. Handle parental, family and guardian contacts as assigned.
  14. Provide orientation and support to SLS and CES providers and contractors' billing designees in the Individual Support Plan process, including assessments, safety and monitoring.
  15. Oversee agency mobile crews and cleaning work crews, which includes developing and maintaining work contracts and contacts with the public to provide paid work for the crews.
  16. Develop and implement a routine and preventive maintenance plan to ensure the cleanliness and upkeep of assigned agency vehicles.
  17. Work cooperatively with other departments, programs, and agencies in the delivery of service.
  18. Provide follow up response to incident reports in a timely and professional manner.
  19. Oversee and assist with staff scheduling in accordance with the agency staffing model and cover vacant shifts, as needed.
  20. Consistently manage the overtime costs of assigned staff to ensure that overtime costs do not exceed the target goal.
  21. Order and maintain necessary supplies, material, and equipment within budgetary guidelines.
  22. Participate in staff training activities and staff meetings, as outlined in the agency's staff development curriculum.
  23. Work multiple sites or work flexible schedules as agency and supervisor deems necessary. Staff attendance and promptness to assigned shifts is essential. The responsibilities of this position may require staff to be able to work irregular, extended, and/or be available for on-call work hours, evenings, nights, weekends, holidays, etc.
  24. Follow agency procedures when purchasing items on behalf of the agency.
  25. Follow agency policies and procedures in carrying out duties.
  26. Advise supervisor of concerns and problems that arise within the agency.
  27. Safeguard all agency funds and property.
  28. Performs other duties as assigned by the Community Supports Director, Adult Services Director, CEO, and/or designee.
  29. Other reasonable duties as may be assigned.

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

MINIMUM/PREFERRED QUALIFICATIONS:

MINIMUM/PREFERRED QUALIFICATIONS:

Minimum: This position requires a Bachelor's Degree and one year of demonstrated supervisory experience; OR an Associate's Degree and three years of demonstrated supervisor experience; OR a minimum of one year (30 credit hours) of college level courses at an accredited Junior College, Community College, 4-year College or 4-year University AND five years of demonstrated supervisor experience; OR a minimum of five years of demonstrated supervisor experience in any industry; OR a minimum of six years of experience working with individuals with intellectual and developmental disabilities.

Computer knowledge and experience with Google Docs, Sheets, and Slides programs. Must be at least 18 years of age. Persons hired into this position must be licensed drivers and have reliable transportation. If current license is from out-of-state, a valid Colorado Driver's License is required within ten days of hire. Background check, drug and alcohol check, reference check, and motor vehicle checks must be passed. Current motor vehicle insurance is required. Driving is an essential job duty. Must be willing to relocate within 30 miles of La Junta, CO within 90 days.


Preferred: In addition to schooling and work experience outlined above, two years of experience working in the field of developmental disabilities.

REASONING ABILITY: Able to deal with problems involving a variety of concrete variables in standardized situations. Able to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Able to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Able to define problems, collect data, establish facts, and draw valid conclusions.

  1. SUPERVISORY RESPONSIBILITIES:

The Program Supervisor is responsible for direct supervision of assigned staff and individuals. The Community Supports Director and Adult Services Director provide supervision of this position. In general, this position requires that the individual work independently approximately 95% of the time.

Able to work with others, with system issues, and able to work within designated time frames. Patience in dealing with staff, individuals in service, and parents. Able to develop effective working relationships with staff, individuals in service, and the community. Good communication skills and able to work with others as a team is a must. Able to communicate clearly and concisely, both orally and in writing. Able to perform duties in accordance with agency policies and procedures.
