The Licensed Clinical Social Worker provides psychosocial assessments, crisis intervention services and counseling to patients. This position coordinates healthcare services throughout the continuum of care on behalf of the patient and assists in discharge planning. Supervises MSW, LSW and LMSW for licensure requirements. This position requires population specific competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. - Current license as a Licensed Clinical Social Worker (LCSW) from the Oklahoma State Board of Licensed Social Workers.
- Experience in a medical setting is preferred.
- Must be able to communicate effectively in English (verbal/written). For Arcadia Trails Center for Addiction Recovery only:
- Prior experience with psychiatric and/or addiction preferred.
- This job may require the incumbent to operate an INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Driver's License from the state of residence, as well as a driving record which is acceptable to our insurance carrier.
The Licensed Clinical Social Worker responsibilities include, but are not limited to, the following:
- Conducts thorough psychosocial assessment of patient (including emotional, cultural and financial needs) utilizing all available resources
- Provides assistance with evaluation and referral in situations of possible abuse/neglect for adult and pediatric patients
- Implements social service interventions based on psychosocial assessment. Counsels patient/family in adjustment to illness, treatment and the process of grieving/conflict resolution.
- Assists patients/family members in obtaining advanced directives and/or durable power of attorney for health care
- Works collaboratively with Case Managers and the health care team in formulating and facilitating patient hospital discharge plans
- Acts as a liaison between patients, family members, physicians, interdisciplinary team and external agencies and organizations making referrals as appropriate. Facilitate meetings for discussion of plan of care and discharge planning.
- Obtain and maintain Oklahoma Licensure Supervisor designation as needed by the department.
- Supervise MSW, LSW and LMSW weekly for their licensure requirements as needed by the department. For Arcadia Trails Center for Addiction Recovery only:
- Provides aftercare case management and monitoring 12-months post-discharge (work, health, recovery status, legal encounters, family and quality of life).
- Conducts follow-up telephone interviews at 3, 6, 9, and 12-month intervals using standardized assessment tools.
- Develops written reports for referents/family members on status updates, as appropriate.
- May facilitate aftercare group sessions and alumni group activities.
- Works with adult patients only.
- Will not supervise staff. Reports to the department manager or director. For Arcadia Trails Center for Addiction Recovery only: Reports to the Assistant Vice President of Arcadia Trails or designated leader.
