POSITION SUMMARY:
The Nursing Assistant, under the direction of the Registered Nurse or Licensed Practical Nurse, is responsible for providing quality patient care, which includes knowledge of physical, psychological, social, cultural, spiritual, and developmental factors of clients served
As a member of the care delivery team, assists the professional nursing staff in providing patient care necessary for comfort, hygiene, simple treatments, data collection, and documentation requiring little or no variance in routine. Participates as a team member by performing additional assignments not directly related to the job description when workload requires and as requested by Charge Nurse. The Certified Nurse’s Aide reports to the Clinical Director
ESSENTIAL JOB FUNCTIONS
· Ensuring a safe and therapeutic environment
- Responsible for providing quality patient care, which includes knowledge of physical, psychological, social, cultural, spiritual, and developmental factors of patients served, is family-centered and based on the philosophy of Integrity Hearts Health Services
- Aid in completing instrumental activities of daily living (IADLs) which includes but is not limited to:
o Meals – including planning and preparation, set-up, storing food, clean up, and washing dishes
o Light housekeeping – including sweeping, mopping, dusting, vacuuming
o Laundry- including washing, drying, folding, and putting away laundry, changing linens and ironing clothes
o Transportation – include direct help with making travel reservations or even drive the patient’s where they need to go
o Aid in completing activities of daily living (ADLs) which includes but is not limited to:
o Grooming - Basic hair care, oral cares, shaving, basic nail care, applying cosmetics and deodorant, care of eyeglasses and hearing aids.
o Bathing - Basic personal hygiene and skin care.
o Eating - Completing the process of eating including application of orthotics required for eating, hand washing and transfers.
o Transfers - Assistance to transfer the person from one seating or reclining area to another.
o Mobility - Assistance with ambulation.
o Positioning - Assistance with positioning or turning a person for necessary care and comfort.
o Toileting - Helping person with bowel or bladder elimination and care. This includes transfers, mobility, positioning, feminine hygiene, use of toileting equipment or supplies, cleansing, inspection of the skin and adjusting clothing
· Making home visits to clients living in designated territories
· Monitoring clients' conditions; reporting changes to Clinical Nursing Director
· Answer client questions about self-care techniques and give instructions that will assist family, client, and family/support in providing care.
· Lift, turn, and weigh clients as appropriate.
· Carry out assigned tasks on clients and follow Home Care Plans.
· Perform delegated tasks after instruction by RN or PT.
· Assist with all aspects of activities of daily living.
· Take and record vital sign checks: temperature, pulse, respiration, blood pressure, oxygen(SPO2)
· Home Management Tasks: laundry, meal preparation, bed making etc.
· Medication reminders
· Transfers /use of mechanical lifting devices
· Activities of daily living including: bathing, dressing, toileting, feeding, range of motion/exercises, ambulation, etc.
· Light housekeeping
· Patient data collection and documentation (i.e. vital signs, intake/output, change in patient condition)
· Provides accurate, precise, timely documentation when applicable for patient care
· Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing medical records
· Ability to practice as an effective member of an interdisciplinary team.
· Able to speak, write, read, and comprehend English
· Ability to travel to cases, as assigned
EDUCATION
· Successful completion of State Approved Nursing Assistant Training and Competency Evaluation Program. Must be a Certified Nursing Assistant in accordance with Virginia state laws.
· A minimum of one year of clinical experience is preferred
LICENSING/CERTIFICATION
· Current certified nurse’s aide licensure in good standing in state where services are performed.
· Basic Life Support (BLS) – American Heart Association
· Must be registered and listed in good standing on the Nurse Aide Registry in the state of employment
· Possess a valid state driver’s license or valid identification card and automobile liability insurance if applicable
