As a Hospice Client Care Coordinator (CCC) , you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Hospice Client Care Coordinator (CCC), you will:
* Facilitate new client enrollment with initial and subsequent screening assessments and further maintain client’s healthcare record
* Identify resources to address client’s health, daily living, and safety needs
* Perform administrative tasks such as scheduling, logging, documenting all appointments and services
* Communicate to clients through telephone, computer, and home visits
* Coordinate with RN and pharmacy medication set-ups and reminders
* Accompany and/or arrange transportation for the client to the provider health care appointments
* Be a family liaison and coordinate with the family, designees, and providers
To qualify to be a Hospice Client Care Coordinator (CCC) with us, you will need:
* Education: High School graduate or equivalent
* Healthcare experience is required
* Customer service experience is required
* Experience in working in the senior field is preferred
* Must have a valid driver’s license
