Job Responsibilities for the Registered Nurse (RN):
- Perform assessments and procedures.
- Contribute to the patient’s plan of care/service plan.
- Coordinate plans for patient discharge from services
- Coordinate care with other disciplines.
Communicate effectively with other patients, families, field staff, office staff and physician offices.
Job Requirements
- Active Registered Nurse (RN) License in Colorado
- Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the state(s) in which he/she practices.
- At least 1200 hours of experience as an RN
- Health care CPR/BLS Certified
- Home health care experience preferred, but not required
