GENERAL POSITION SUMMARY:
IRCO Mid-County District Center is a contractor with the Multnomah County Department of County Human Services, Aging, Disability, & Veteran Services Division (ADVSD) to provide services for older adults and people with disabilities living in Multnomah County. The Service Plan Coordinator provides services to help clients remain independent, delay, or prevent out-of-home placement, and support client self-determination. The Service Plan Coordinator will work in collaboration with community partners and will provide a broad array of services for people with disabilities and seniors, including, but not limited to Service plan Coordination, Long-term and Short-term Case Management, Options Counseling, Information and Referrals assistance, health and wellness, and focal point leadership.
ESSENTIAL FUNCTIONS:
The Service Plan Coordinator is responsible for providing conflict-free case management, independent of eligibility. This position provides person-centered service planning for and with eligible individuals, including:
- Authorizing and implementing an individual's service plan and monitoring service provision.
- Providing limited, non-medical assistance to delay or prevent the need for full Medicaid long term services and support.
- Optimizing eligible individual personal resources and natural support.
- Providing access to limited, preventative in-home services to more older adults and adults with disabilities who need assistance with daily activities.
- Providing risk assessment and monitoring, including, Identifying and documenting risks; working with an individual to eliminate or reduce risks; developing and implementing a Risk Mitigation Plan; monitoring risks over time; and adjusting an individual's person-centered service plan as needed.
- Other program coordination such as helping an individual navigate or coordinate with other social, health, and assistance programs.
- Crisis response and intervention, such as, assisting an individual with problem resolution.
- Authorizing services to meet the service choices, assessed need and preferences of the individual considering natural supports and other alternative service resources.
- Service provision issues such as assisting an individual with problem solving to resolve issues that occur with providers, services, or hours that do not meet the individual's needs, and documentation of changes in condition, service plan adjustments and reasons for adjustments and exceptions, completion of required forms and other service plan case management requirements.
- Providing conflict-free case management services. Conflict-free case management protects against conflicts of interest, fraud, and misuse of funding to benefit a provider.
- Making and monitoring quality referrals and collaborating across agency lines to guide client towards independent living within the context of available resources and creative problem solving.
- As required and depending on education and experience, the Service Plan Coordinator could also be trained to lead an additional program related to Case Management such as STAR C or other activities as needed.
Requirements
JOB SPECIFIC QUALIFICATIONS:
- Associate degree or equivalent in social services or related field.
- Minimum of 2 years experience working as a case manager in social services programs. Familiar with case management practices i.e., interviewing, and listening.
- Demonstrated ability to work effectively, independently, and as part of a cross-disciplinary team.
- Ability to research, analyze, and organize information about community resources and mobilize and leverage resources to encourage a client to stay independent and not move to an assisted living facility.
- Bilingual: written and spoken in a language in addition to English is preferred, but not necessary.

Education: Associate's Degree (3)
Experience: 1-2 years of experience is required (3)

Computers: All positions at IRCO require computer use and experience working with Microsoft Products including Microsoft Office 365, Excel, Word, Outlook, and PowerPoint. In addition to Microsoft products, this position specifically requires the use of State, Multnomah County, and IRCO agency software and programs.
