PT - Physical Therapist

• Graduated from an accredited Physical Therapy (PT) college or university
• Unencumbered Physical Therapy (PT) license for New York
• Conduct home visit Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation
• Proficient in EMR documentation (we use Hello Note but experience in this system is not required)
