You will be responsible for assisting the RN Case Manager in carrying out the plan of care for your assigned patients on an intermittent basis in their place of residence.

A brief overview of Responsibilities:
- Under the direction of an RN, assist with the preparation, implementation and continuing evaluation of the patient care plan.
- Recognize and understand the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients and their families.
- Prepare clinical and progress notes based on patient visits.
- Performs an ongoing assessment during each visit and documents data
- Observe, record, and report to the appropriate person patient symptoms, reactions, and changes, as appropriate.
- Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders.
- Participates in on-call duties as defined by on-call policy
- Other duties as assigned
Qualifications:
- Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
- At least one year of hospice experience required.
- Graduate of an accredited nursing school.
- Must have a valid driver's license and a reliable car available during work hours.
