Job Summary
The position will provide behavioral health assessments, brief evidenced-based interventions, clinical/consultative services and education using the Primary Care Behavioral Health (PCBH) model, for patients and their family in Jefferson ambulatory practices of varying specialties. The Behavioral Health Consultant (BHC) in this role will serve patients ranging in age from toddler through late adulthood. The BHC will also provide consultation with the Primary Care Provider (PCP) to enhance the understanding of the patient, provide decision support for treatment planning, and assist in the ongoing care and monitoring of patients’ behavioral health care and biopsychosocial treatment plans. Ensures behavioral health services are delivered in a culturally appropriate fashion and is consistent with State laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines. Essential Functions
Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
Integrates evidence-based practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal health care.
Partners with patients, their family/designated care partner(s) in identifying learning needs and priorities and provides education in ways that are useful, understandable and affirming.
Assist primary health care providers in recognizing, treating and managing patients with chronic emotional, psychosocial, substance use and/or physical health problems within the primary care setting.
The BHC will provide functional strengths-based assessments, psychoeducation, brief problem-solving cognitive intervention, medication adherence counseling, and disease self-management counseling among others. Triaging and referring patients to specialty mental health care when appropriate.
Provides clinical and administrative oversight to assigned IBH – BHCs.
Assists with managing schedule templates for assigned BHCs
Provides direct 1:1 and group supervision to assigned BHCs
Completes chart audits as directed for assigned BHCs
Assists in interviewing, onboarding, and training for new BHCs
Competencies (Knowledge, Skills, and Abilities Required):
Knowledge of: Medical terminology and disease process; documentation best-practices
Skill in: Work with multidisciplinary teams, patients/families/significant others
Ability to:
Establish an appropriate plan to safely discharge patients to the next level of care as well as understand and adhere to city, state, and federal regulatory requirements.

Minimum Education and Experience Requirements:
Education:
Master’s in social work from a School of Social Work accredited by the Council on Social Work Education or an equivalent degree as determined by the Council on Social Work Education; or PhD/PsyD in Psychology at an accredited university or college.
AND
Experience:
A minimum of three years of experience in a behavioral health setting is required.
A minimum of one year in a supervisory role is required.
Minimum Certifications, Registration or License Requirements:
Licensed Clinical Social Worker (LCSW) or Licensed Clinical Psychologist (PhD; PsyD)
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
