JOB TITLE: Social Worker
REPORTS TO: L’Chaim Program Director
EMPLOYMENT TYPE: Part-time
JOB LOCATION: San Francisco, CA

WHAT YOU’LL DO

  • Responsible for the coordination of Social Services for participants as specified by ADHC, CBAS, Medi-Cal Title XXII(54309,54329 &78339), Managed Care Plans, and the California Code of Regulations
  • Implement the Home and Community-Based Setting (HCBS) waiver 42 CFR 441.301 person-centered care planning requirements
  • Coordinate with the California Department of Aging (CDA) to prepare for the Medi-Cal Licensing Recertification Site Survey
  • Conducts an initial assessment and subsequent reassessments (at a minimum of every six months) of each ADHC participant for social services and monitors implementation
  • Prepares treatment plan and Individual Plan of Care (IPC) for each participant
  • Ensures IPC specifically address the full range of participants’ impairments, detail necessary treatment, and establishes appropriate, attainable, and measurable outcome goals
  • Ensures that social service care plans are coordinated with nursing, physical therapy and occupational therapy care plans and fully address the social service needs of clients within the scope of work of a licensed Adult Day Health Care
  • Reviews monthly flow sheets and quarterly reassessments provided by the Social Worker Assistant, ensuring that the full scope of services established in the IPC and TAR are provided and that the treatment interventions and outcomes are monitored and documented
  • Ensures that participant’s functional and cognitive disabilities are thoroughly documented and that all treatment is fully and accurately recorded
  • Provides individual supportive counseling to participants as recommended in their treatment plans
  • Provides case management to participants as recommended in their treatment plans
  • Discharges participants if they are inappropriate for ADHC services; assists caregiver with discharge plan
  • Provides referral for persons not appropriate for adult day health care program.
  • Provides information and referral for community members and for all ADHC participants
  • Reports suspected verbal, physical, fiduciary, and/or sexual abuse to appropriate monitoring agencies
  • Develops and maintains positive working relationships with human service agencies, volunteers, and community members

QUALIFICATIONS

  • Bachelor’s Degree in social work from an accredited school of social work with two years of experience providing social services in one or more of the fields of aging, health, or long term care services, OR Master’s Degree in Social Work, Psychology, Gerontology, or counseling with one or two more years of experience providing social services in one or more of the fields of aging, health, or long-term care services, or licensed by the California Board of Behavioral Sciences
  • Proven experience assessing the social service needs of frail elderly and developing care plans to address needs in a manner that enables measurement of outcomes
  • Previous experience in effective case management services to frail seniors
  • Experience providing services in an Adult Day Health Care or comparable environment
  • Certifications: CPR, First Aid within three months of hire
  • Proven ability to document care plans, interventions, and outcomes in an effective manner that meets professional standards for documentation of social services
  • Excellent oral and written communication skills
  • Russian language conversational skills a plus
  • Ability to communicate appropriately and effectively with participants, professional staff, families, and caregivers
  • Ability to work independently as well as on a team
  • Ability to effectively utilize Microsoft Office Suite software including Outlook
  • Physically capable of performing the required duties and of assisting frail elderly and disabled adults
  • Must have received or be willing to receive the COVID-19 vaccination by the date of hire to be considered
