SUMMARY
The Residential Coordinator is responsible for managing and performing consumer plan development and staff supervision in the quality care, treatment and delivery of residential services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conduct/participate in the interviewing, hiring, evaluation, and disciplinary action of employees
- Supervise and manage assigned residential direct care staff/caregivers in the care of consumers to include, but not limited to, the following:
- Conduct staff/caregiver meetings and provide necessary group or individual training
- Conduct on-the-job training for new employees
- Inspect employee duty assignments regularly
- Follow-up with employees/caregivers regarding mandatory meetings/trainings
- Review employee and consumer purchases and provide corrective action where necessary
- Review Fire Drills and Disaster Drills and provide corrective action where necessary
- Check Medication Administration Records for accuracy
- Conduct announced and unannounced visits of assigned residences/homes when employees/caregivers and consumers are present to include:
- At least 1 weekend day per month per assigned residence/home
- At least 1 week day visit during 1st or 2nd shift hours per assigned residence/home, minimally
- At least 1 third shift visit per quarter per assigned residence
- Coordinate/conduct activities with appropriate employees/caregivers to correct problems discovered during inspections and reviews
- Perform treatment and care planning activities for assigned consumer to include, but not limited to, the following:
- Coordinate the completion and revision of comprehensive functional assessments
- Monitor consumer progress and staff/caregiver implementation of program plans monthly
- Revise objectives and program plans
- Prepare Called Team meeting notes
- Review health and behavior support reports and follow-up on necessary interventions
- Coordinate treatment plan changes, consumer issues/concerns with appropriate staff to include, but not limited to, Direct Care Staff/ Caregiver, Case Manager, Day Services Manager, Behavior Support Consultant, Psychiatrist, Nurse, etc
- Coordinate scheduling respite for Caregivers (CTHI)
- Review time off requests and coordinate scheduling needs before approving the request
- Cover shift when appropriate/timely coverage cannot be located
- Assist in coordinating medical and psychiatric services/appointments with Nursing Department
- Perform, or coordinate staff, in training activities (shopping, checking meds, checking finances, etc.) with consumers receiving SLP I services, as assigned
- Attend and participate in the other Departmental annual and called plan meetings of assigned consumers
- Review consumer fund balances, expenditures, and personal property inventory
- Complete Consumer Financial Plan annually
- Coordinate activities to maintain Medicaid eligibility of consumers
- Review and finalize General Event Reports and conduct follow-up activities
- Maintain ongoing progress notes of consumer and family events
- Maintain records in software provided by Agency to ensure HIPAA compliance
- Review/analyze trends available in various software programs (Personnel and Consumer)
- Interact with consumers to acquire their satisfaction with services, needs, etc
- Keep families informed of significant events
- Obtain Informed Consent for restrictive programs and non-medical informed consent from consumer and family, forward consents to appropriate staff
- Participate in and coordinate training of Behavior Support Plans
- Prepare behavioral information for Psychotropic Drug Review and Human Rights Committee Meetings
- Carry phone, at all times, for emergency and after-hour critical issues
- Assess critical behavioral incident and inform agency nurse of situation to request medication evaluation if situation cannot be resolved by other means
- Participate in Residential Department Staff meetings, committee meetings as assigned and attend after-hour events that support activities of the Agency
- Perform assignments during Agency crisis and weather emergencies
- Assist consumer movement decisions; Prepare and review admission packet with consumer and family
- Prepare and submit agency purchase orders to Residential Director
- Submit weekly schedule to Residential Director
- Other related duties as assigned
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
- Excellent written and oral communication skills
- Computer literate using a variety of available software and programs
- Ability to perform a variety of tasks, often changing assignments with short notice
- Willingness to work flexible hours and locations

- Good physical abilities to include: fine and gross motor skills necessary to be able to perform the job, excellent verbal communication (speaking and hearing), visual acuity (reading to include small print) and mathematical concepts
- Subject to both indoor and outdoor environments and may be subject to temperature of hot and cold conditions for short periods
- May be subjected to possibilities of physical assault, property destruction and may be required to wear protective clothing to prevent infections
EDUCATION AND EXPERIENCE REQUIREMENTS
BA/BS degree in human services from an accredited college or university with three years of experience in the field of intellectual disabilities and two years of experience in supervision.
